I give my child permission to attend and participate in the Felix Wrestling Academy practices. I understand that his or hers participation in this involves risk and dangers that could result in serious bodily injury. I also understand that the Felix Wrestling Academy LLC. and its staff will not assume any responsibility for any accidents, medical or dental, or any other expenses incurred as a result injury during these practices. I verify that my child has medical insurance and a USA Wrestling card and is physically able to participate at the Felix Wrestling Academy. I agree to allow my child to be treated by a certified coach or trainer while attending (if necessary). Waiver-Concern for COVID-19. At the Felix Wrestling Academy, we assure a safe clean environment. The wrestling mats are sanitized before and after every practice. I assume all risks and take all responsibility by agreeing to this signed waiver. I understand there are risks involved including COVID-19 while attending the Felix Wrestling Academy. If my child becomes sick or tests positive for COVID-19 I will take complete responsibility and place no blame on the Felix Wrestling Academy LLC. or its staff. During this time all parents/spectators must wear a mask while inside the FWA facility.
